PLEASE READ ENTIRE APPLICATION CAREFULLY!

Make sure to submit ALL documentation along with the application (not piece by piece).

The USVI Health Department requires you to fill out extra forms to acquire a tattoo license if you’re a Non Resident.

These forms must be digitally filled out (CANNOT BE HAND WRITTEN) and submitted to the USVI Health Department.

Do not physically mail the Temporary Tattoo License! Please EMAIL the finalized form to Ramona.Liger@doh.vi.gov

If you are a returning artist please check your tattoo license renewal date and please submit any paperwork that may need to be updated to Ramona.Liger@doh.vi.gov and CC CalypsoTattooFest@gmail.com to keep your license in date.

Do not fill out the tattoo license from again if your license is up to date.
